When learning navigation I was taught, "When two ships are in sight of one another, there is a chance for a collision."
Same applies to people handling a weapon, except the numbers are cut in half! A along time member of my local club I can say we too have the same problems but just not on the same level as the general public ranges. You have to be on-guard all the time you are handling a firearm and be diligent to what others around you are doing as well.
Training is only half the game.
